Improving HealthCare coordination in New Orleans

In this position, I was the Project Manager of a multicultural team to find solutions in implementing a care coordination model in the Greater New Orleans Area for an organization created by a $13.5 million dollar national grant.

LPHI is a non-profit institution working with both public and private entities such as the government, hospitals and clinics, health insurance companies, as well as medical patients.

The purpose of this consulting project was to develop a business model allowing the creation of a behavioral health program within Louisiana to avoid redundancies in the admission of chronically-ill patients to the Emergency Room.

The quality work we provided allowed us to find innovative and unconventional ways of improving current Health Care system efficiency through health information technology and the potential outcomes of patients’ healthcare, while decreasing the overall average cost of admission.

Professor Sherif Ebrahim supervised the project and has been recognized for his work in healthcare and strategic leadership by two U.S. Presidents, the Secretary of the U.S. Department of Health and Human Services and several companies.
